Patrick M. Chamberlain, 29, of South Bend, Indiana passed away on Friday, June 12, 2015 in his home. He was born on February 22, 1986 in South Bend, Indiana.

Patrick was a graduate of Riley High School.

He is survived by his father, Barry Chamberlain of South Bend, Indiana.

Graveside Services will take place at 10:00 a.m., Thursday, June 18, 2015 in Riverview Cemetery. Kaniewski Funeral Home is in charge of arrangements.
